The maintenance dose in patients with kidney failure depends on the patient kidney function and the fraction of drug dose excreted unchanged in urine.

Dosefailure = Dosenormal [1 - f(1 - KF)]

The dose for renal failure patients should achieve average s.s. conc similar to the s.s. conc achieved after giving the full dose to patients with normal kidney function.

Example: What is the dose of procainamide required in a patient with 50% of normal kidney function? (f = 0.6, and the normal dose is 500 mg q 6 hr)

Answer:

Dose failure = 500 mg [1 - 0.6(1-0.5)]

Dose failure = 350 mg q 6 hr

is similar in both cases, however
the maximum and minimum
