- Basic and clinical pharmacokinetic knowledge
Procedures :
1- Obtain information about the patient (demographic and past medical history).
2- Use the available patient information to design an appropriate initial dosing regimen for the patient.
3- If the patient already receiving the drug to be monitored, obtain the dosing history.
4- Plan an appropriate drug monitoring strategy including plasma sampling schedule that allow the determination of the patient specific pharmacokinetic parameters for the drug.
5- Calculate the patient specific drug pharmacokinetic parameters.
6- Use the patient specific pharmacokinetic parameters to adjust the drug dosing regimen, if necessary, to achieve the desired therapeutic drug concentration.
7- Develop drug therapy management plan that include periodic monitoring of the plasma drug concentration.
