Individualization of aminoglycoside dosing regimen

(Gentamicin, tobramycin, netilmicin, and amikacin)

Aminoglycosides are antimicrobial agents that have bactericidal activity against Gram-negative aerobic bacteria.

Individualization of aminoglycoside dosing regimen is important to ensure optimal efficacy with minimal toxicity. The goal of conventional aminoglycoside dosing adjustment strategies is to maintain the maximum and the minimum plasma conc within a certain range.

Guidelines for plasma aminoglycoside conc for most infections:

Gentamicin, tobramycin and netilmicin Amikacin
Maximum conc 6 - 10 mg/L 20 - 30 mg/L
Minimum conc 0.5 - 2 mg/L 1 - 8 mg/L

Once-daily dosing of aminoglycosides has been utilized effectively in the treatment of infections. The above guidelines do not apply to the once-daily dosing method. The following discussion will cover the traditional aminoglycoside dosing adjustment method.
