ii - Selection of dose

The dose and infusion time (Ko=Dose/t') required to achieve the desired maximum steady state conc when given every dosing interval (τ ) can be determined from:

Again it may be necessary to round the calculated aminoglycoside dose to the nearest 5 mg to allow accurate dispensing.

It is usually necessary to calculate the
expected maximum and minimum plasma conc
from the recommended dosing regimen.
This is calculated by substituting the dose,
infusion time and dosing interval in the
equations for the maximum and minimum
plasma conc
. This helps to determine the extent
of deviation from the desired conc.

iii- Selection of the loading dose

When administration of a loading dose is necessary, the loading dose should be selected to achieve plasma conc (Cp) that is equal to the desired Cpmax ss
