Drugs that follow multi-compartment pharmacokinetic models can have their site of action in equilibrium with the drug in the central compartment or the drug in the peripheral compartment.

This can be determined by examining the drug conc-time profile and the drug effect-time profile.

This effect-time profile indicates that
the drug in the peripheral compartment
is in equilibrium with the drug at
the site of action.

The pharmacokinetic model can be used to estimate the drug conc in the peripheral compartment. Then the drug conc in the peripheral compartment and the observed effect can be used to determine the best pharmacodynamic model that describes the drug conc-effect relationship. The model parameters can be estimated from the drug conc in the peripheral compartment and the observed effect.
