Effect of the change in intrinsic clearance
on the hepatic extraction ratio

The hepatic intrinsic clearance can change due to enzyme induction which results in increasing the amount of enzymes available for drug metabolism, or due to diseases that can affect the liver function and decrease the liver ability to metabolize the drugs.

The increase in intrinsic clearance results in increasing the hepatic extraction ratio.

Notice that at lower values of intrinsic clearance (low extraction ratio drugs) even small change in intrinsic clearance results in significant change in extraction ratio.

However at higher values of intrinsic clearance (high extraction ratio drugs) changes in the intrinsic clearance results in very small change in extraction ratio.
