Effect of hepatic blood flow and intrinsic clearance on hepatic clearance

The hepatic clearance is dependent on the intrinsic clearance and the hepatic blood flow.

a- For drugs that have CLint which is much smaller than the hepatic blood flow (i.e. Q >> CLint, low extraction ratio drugs) The above relationship can be reduced to:

This means that the hepatic clearance for low extraction ratio drugs (E is very small) is dependent on the drug intrinsic clearance and any change in enzyme activity such as enzyme induction or inhibition will have significant effect on hepatic clearance.

b- For drugs that have CLint which is much larger than the hepatic blood flow (i.e. CLint> > Q, high extraction ratio drugs) The above relationship can be reduced to:

This means that the hepatic clearance for high extraction ratio drugs (E is very large) is approximately equal to the hepatic blood flow and any change in hepatic blood flow will have significant effect on the hepatic clearance of these drugs.
